After Registration process the following screen will be open. Now you have to check the Declaration and press Save Button. In the declaration you are agree that “You are requested to provide all information truthfully to the best of your knowledge. In case any information provided by you is proven to be falsely reported at the time of registration/survey, your family may be barred from availing any benefit under the social protection or welfare programs of the government.”

The Head of Family have to enter his information first and after that enter The information of all Family Members according to NADRA Family Record, one by one. Fields marked with an asterisk (*) are required and the data will not be saved without them.
